Public Health Vaccination Clinic

Dear Parent/Guardian,

Public Health would like to take this opportunity to invite your child to a Covid19 Vaccination Clinic to be held at Bathurst High School on September 21, 2021.

Vaccination for those born in 2009 or later is strongly encouraged as vaccination is the most effective way to protect yourself, your family, and your community from the Covid19 virus.

Students are invited to attend this clinic for their first or second dose of a Covid-19 vaccine. The vaccine to be offered will be Pfizer. For students who will be requesting their second dose of Covid-19 vaccine, a proof of vaccination is required from their initial vaccination.

A Covid-19 consent form can be printed and completed by students aged 16 years and older. For those students under the age of 16 years, a parental/legal guardian signature is required on the consent form.
